DOMINICK’S & UNITED AIRLINES MILEAGE PLUS® PROGRAM

Exclusively for Dominick’s Fresh Values Card Members - earn United Airlines Mileage Plus miles every time you use your Dominick’s Fresh Values Card at participating Dominick’s stores, starting January 1, 2000, through December 31, 2007.

Earn 125 miles on your Mileage Plus account every time you spend $250 in eligible purchases through December 31, 2007 at participating stores. Restricted or excluded items will not be eligible toward spending accumulation totals. All dollar amounts are rounded down.

If you are a Dominick’s Fresh Values Card member and a United Mileage Plus member, sign up at a participating Dominick’s store.  Complete a Dominick’s Fresh Value Club Card application form. To be eligible, you must submit a fully completed and accurate application, even if you are already a Dominick’s Fresh Values Club Card member.  You will want to enter your existing Club Card number under Changes to Existing Card Account, check the box Add United Mileage Plus Account Number and complete Section 1.  You will need your Mileage Plus account number to enter on the application for enrollment in the program.  Or you can sign up online atwww.grocerymiles.com, you will need both your Dominick’s Fresh Value Club Card number and your Mileage Plus account number.  Go to www.grocerymiles.com to enroll.  If you do not remember your Dominick’s Fresh Value Club Card number call customer care at 1-877-723-3929.

If you are a Dominick’s Fresh Values Club member, but not currently a United Mileage Plus member, obtain a Mileage Plus account number at the United Web site (www.ual.com), or by calling 1-800-421-4655.  Then complete the Dominick’s Fresh Values Club Card application at your local store. You will want to enter your existing Club Card number under Changes to Existing Card Account, check the box Add United Mileage Plus Account Number and complete Section 1. You will need your Mileage Plus account number to enter on the application for enrollment in the program.  Or enroll online at www.grocerymiles.com you will need both your Dominick’s Fresh Values Club Card number and Mileage Plus account number.

 If you are not a Dominick’s Fresh Values Club member, but are a United Mileage Plus member, complete a Dominick’s Fresh Values Club Card application, and enter your Mileage Plus account number in the space provided under Section 1 of your application.  Turn in your completed Club Card application at your local Dominick’s store making sure your information is current.

If you are neither a Dominick’s Fresh Values Club member nor a United Mileage Plus member, you will need to first enroll in the Mileage Plus program at the United Web site (www.ual.com), or by calling 1-800-421-4655. Then complete a Dominick’s Fresh Values Club Card application and enter your new Mileage Plus account number in the space provided under section 1. Turn in your completed Club Card application to your local Dominick’s store, making sure your information is current.  Or if you obtain your United Mileage Plus account number and your Dominick’s Fresh Values Club Card number you can then enroll online at www.grocerymiles.com.

For this promotion, Mileage Plus miles are accumulated on a per household basis (maximum of 10 people to a household). As a result, all Mileage Plus miles earned through this promotion will be credited to a single Mileage Plus account number designated in Dominick’s records.

How to Qualify: Every time a Dominick’s Fresh Values Club member uses a Dominick’s Fresh Values Club Card at a participating store, through December 31, 2007, the Dominick’s Fresh Values Club member's household (see above) accumulates dollars toward earning Mileage Plus miles. For every $250 spent on eligible purchases, the Dominick’s Fresh Values Club member’s household earns 125 Mileage Plus miles. Excluded items are not eligible toward spending accumulation totals.  Accumulations exclude purchases of Liquor, Tobacco, Fuel, US Postage Stamps, Fluid Milk, Bus/Commuter Passes, Fishing/Game Licenses, Ski Tickets, Lottery Tickets, Gift Certificate sales, Pharmacy prescriptions, Fresh Values Card Savings, Dominick’s Store Coupons, and Sales Tax.

Accumulated dollar amounts for eligible purchases will print on the Dominick’s Fresh Values Club member's receipt tape. When the accumulated total reaches $250, the award message will indicate that 125 Mileage Plus bonus miles have been earned. From the date of application, Dominick’s Fresh Values Club members should allow at least 4-6 weeks for their register receipt tapes to reflect dollars accumulated toward bonus mile awards. For each household applying for this promotion between January 1, 2000 and February 14, 2000, dollars from eligible transactions will be accrued from January 1, 2000. For each household applying after February 15, 2000, dollars from eligible transactions will be accrued from the date of application, provided that the applications are fully completed, accurate, and submitted in accordance with these rules. For households applying on or after January 1, 2000, in no event will transactions occurring before January 1, 2000 count toward earning Mileage Plus miles.

Mileage Plus miles are earned in increments of 125 bonus miles with a spending level of $250 of eligible purchases. If a Dominick’s Fresh Values Club member's eligible spending does not reach $250 during the promotional period, no miles will be earned or awarded. All earned Mileage Plus mile award totals will be transferred on a regular basis to United Airlines Mileage Plus by Dominick’s through December 31, 2007. All accumulated miles will be officially reported to Mileage Plus members by United Airlines Mileage Plus. Please allow 4-6 weeks for miles to be posted by United Airlines Mileage Plus. Store register receipt tapes may not fully reflect a Dominick’s Fresh Values Club member’s accumulated dollar amounts if a household member shops in multiple participating stores within Dominick’s Division. All inquiries about Mileage Plus accounts and actual account balances should be directed to the Mileage Plus Service Center at 1-800-421-4655 or the United website at www.ual.com.

All Mileage Plus miles earned and awards issued, and the use of either, are in accordance with the terms and conditions of the United Airlines Mileage Plus program. Not valid with any other offers. United Airlines, its subsidiaries, affiliates and agents are not responsible for any food or drug products associated with this offer. Dominick’s and its affiliates are not responsible for any airline travel, or for lost, late, damaged, delayed, destroyed, misdirected, incomplete, illegible or postage due applications or mail, or Dominick’s Fresh Values Club cards scanned incorrectly, or scanner/computer failures or malfunctions of any kind, or human or technical errors or failures in the accumulation of dollars or miles, or in the transmission or receipt of applications.

Dominick’s reserves the right, at its sole discretion, to cancel this promotion if it becomes technically corrupted such that it cannot be conducted as originally planned, and to alter or change the excluded items at any time and without individual notice to Dominick’s Fresh Values Club Members. Dominick’s also reserves the right to verify the eligibility of purchases and the household status of purchasers (such that accumulations may be limited to members of a group of individuals who are living as one economic unit and sharing living expenses). Dominick’s and United Airlines Mileage Plus each reserve the right to terminate or suspend accumulations or membership due to fraud or misuse.

Any unspecified expenses related to acceptance/use of Mileage Plus miles are solely the Dominick’s Fresh Values  Club member's responsibility.
